If you are visiting New York Surgical for the first time, please plan on arriving 15 minutes prior to your scheduled appointment. This will allow our team to gather the necessary information needed at the time of registration.
All patients are required to fill out the following forms electrically via e-tablets upon arrival.
Please bring identification, your insurance and a list of all current prescription medications.
At New York Surgical we value our patient's time, and strive to run on time. We intentionally do not overbook our schedule to ensure the best experience for your visit. If for any reason you need to cancel your appointment, please notify the office 24 hours in advance. In the event that you are unable to provide sufficient notice of a change in your appointment, you may be assessed $50 cancellation fee.
